Finding a Network Healthcare Provider

Choose any provider office from our list. Once a member, you may receive treatment from any participating provider. Please note some offices have multiple dentists and some dentists see patients in multiple offices. Detailed information about individual doctors and their offices can be obtained by calling the office directly. Network is subject to change without notice. Call (303) 649-9311 for network updates. Always verify with your dentist that they still participate in the plan type you have purchased.

If a member wishes to transfer from one office to another, please notify ComHealthcare.com by e-mail, letter or telephone. This allows us to provide your new dental office with important eligibility and billing information. All outstanding balances must be cleared.
